The Harbourview Family Health Team offers high quality primary health care for its patients.  An inter-professional group of health care practitioners, including family physicians, nurses practitioners, registered nurses, dietician, and social workers. We provide health care in an integrated, respectful and holistic environment.  The focus of the family health team is chronic disease management and prevention as well as health promotion.  By working collaboratively, the team is able to provide a broad spectrum of Ministry of Health funded, primary health care services to its patients to ensure they receive the care they require.

Family Health Teams are an important part of changing both the health care system and the way people think about health care.  Family Health Teams focus on illness prevention and health promotion.  The teams looks at the best ways of managing chronic illnesses such as hypertension, diabetes and mental health.  In doing so, they are able to provide care that is more accessible and comprehensive and gives patients the opportunity to be more involved in making decisions about their own health.
